The Jefferson County Office for the Aging and NY Connects will offer a six-week workshop on chronic disease management from 2 to 4:30 p.m. Tuesdays at Flower Memorial Library, 229 Washington St., beginning Sept. 17.
People who have chronic conditions, such as diabetes, arthritis, high blood pressure, heart disease, chronic pain or anxiety, are encouraged to attend the Chronic Disease Self-Management Program. Caregivers also are welcome.
Topics include communication, appropriate use of medications, nutrition, exercise, problem solving for health care and how to evaluate new treatments.
Space is limited, so preregistration is requested by calling the county office at 785-3191.
