A Massena licensed practical nurse and a Potsdam pharmacist face disciplinary action from the state Education Department’s Office of the Professions for misconduct.

According to the state department’s website, Moriah K. Crosby, Massena, agreed to a two-year stay suspension, two years probation and a $250 fine for admitting her unauthorized use of prescription medication while at work. Lawrence R. Wheelock, Potsdam, will surrender his license because he admitted to second-degree forgery.

While the website did not provide specifics regarding either matter, it showed Mr. Wheelock faced license suspension and probation in 2002, when he was located in Endicott, after he had admitted to being convicted of petit larceny twice.
